Payment Policy

MilkWagon Breastfeeding Services

Clients are responsible for payment of services at the time of your visit. Patients will be required to keep a credit card on file as a guaranteed form of payment, however, you will have the flexibility to use cash, credit card, debit card, or FSA/HSA card to pay for your visits.

Medical Insurance:

MilkWagon Breastfeeding Services, LLC does not participate in Medicaid or with any insurance companies. I will provide you with a Superbill for both you and your baby, suitable for submission to your insurance company for reimbursement. The Superbill, which also serves as your receipt of payment will be coded appropriate to the level of service provided during your visit. It is not the responsibility of MilkWagon Breastfeeding Services, LLC to know what lactation services are covered by your health plan and cannot guarantee you will receive a reimbursement.

Fees for Multiples:

There is an additional fee of $50 for breastfeeding consults involving twins.

Travel Fees (In-Home Visits Only):

If you have scheduled a home visit, travel fees will apply and are not eligible for reimbursement by your
insurance company. Travel fees are as indicated below:

  • $25 (<15miles from office location)
  • $40 (16-30miles from office location)
  • $55 (31-60 miles from office location)

Cancellation Policy:

Clients cancelling an appointment with less than 24 hours notice will incur a $25 fee charged to your credit card on file. There are exceptions when emergencies arise.

No Show Policy:

No shows for telehealth or in-office appointments will incur a $50 fee charged to your credit card on file. For home visits, if you are not home at the scheduled appointment time with a grace period of 15 minutes, you will be charged $50 plus travel fees to your credit card on file.
